Request a Report

Step 1.Fill Out Records Request Form

Fill out records request form below.  Include the mandatory information:

  • Report number, or location/date/time of incident
  • Name of person(s) involved to include their date of birth or social security number
    • If you represent a person involved, indicate the relationship between the involved people. See Fee Schedule tab for fees relating to this type of request.
  • Reason for request – select and complete the form that matches your request (Commercial or Non-Commercial)

PublicDataRequest-NonCommercial(PDF, 187KB) - This type of request is made for personal use, such as obtaining a police report for your own records, insurance, court matters, or personal reference.They are not used to make money or for business purposes. 

Public Data Request Commercial(PDF, 296KB) - This type of request is made for business or profit-related purposes, such as background research, data collection, media use, marketing, or resale of the information. See Fee Schedule tab for fees relating to this type of request.

Step 2.Payment

See Fee Schedule Tab above.

  • Cash (in person ONLY and will need to be the exact amount)

  • Money Order/Cashier’s Check
  • Credit cards accepted only for online requests by clicking AllPaid . 
    • Instructions: Select make a payment on the home screen, type YAVAPAI, select the option that has PLC code of “a0044j” (do not select the bail bond option), and continue following the directions to submit the credit card payment. Make sure to include the invoice number WITHOUT the dash that is on the invoice that will be sent to you for payment. Once paid, your payment and request will be processed and sent within a few business days.

*Fees are based on per video-hour reviewed and limited per ARS39-129. Fees are per incident. Except as provided in section 39-

127, a county, a city, a town, or any political subdivision of this state may establish a one-time fee per copy, not to exceed $46.00

per video-hour reviewed, that is charged to a person who submits a public records request to a local law enforcement agency

for a copy of a video recording. A county, a city, a town, or any political subdivision of this state may take into consideration the following information when determining the amount of the one-time fee per copy.

1. The reasonable cost of reviewing, transmitting, making a copy of, and, as necessary, redacting the video recording.

2. Any other relevant information.

Get Accident Forms Online

Yavapai County Sheriff’s Office now offers accident reports online. Motor vehicle accident reports, written by the Yavapai County Sheriff’s Office, are available for purchase by clicking on the following link:

Reports are currently available as of March 1, 2010. Some accident reports may not be available on-line as a result of the following:

  1. A Yavapai County Sheriff’s Deputy did not respond to the scene of the accident.
  2. The accident occurred on private property.
  3. An arrest was made.
  4. The report is not approved for dissemination.

Reports are typically available five (5) to ten (10) business days from the date of the accident. If your report is not available within fifteen (15) business days, please contact the Support Services Division, Records Section at (928) 771-3260.

Requesting a background check:

  • In person: Local background check can be completed by visiting one of the YCSO locations listed above. You will need to provide your name, date of birth, and a copy of a valid photo ID. The checks will be conducted in person ONLY. 
  • State Level Check: To request a complete criminal history, click Records Request | Department of Public Safety and follow the Arizona Department of Public Safety website.

 

Important Qualifiers:

  • Records personnel are not authorized to conduct general searches on individuals.
  • Records personnel are not authorized to conduct personal criminal history inquiries through NCIC.
  • Active reports are NOT released by the Yavapai County Sheriff’s Office.
  • All reports are redacted in accordance with state and federal regulations.
